The Think Fast PT website is a virtual physical therapy triage center site. By using the Think Fast PT website, you consent to the data practices described in this statement.
Collection of your Personal Information
In order to better provide you with products and services offered on our Site, Think Fast PT may collect personally identifiable information, such as your:

- Confirmation of Residence: State of NJ or NY

All personal health information will be collected through HIPAA-compliant, secure online services through Google G-Suite, Doxy.me, and Zoom. Collection of private health information will be for the sole purpose of carrying out your physical therapy evaluation and treatment(s).
If you purchase Think Fast PT's products and services, we collect billing and credit card information. This information is used to complete the purchase transaction.
We do not collect any personal information about you unless you voluntarily provide it to us. However, you may be required to provide certain personal information to us when you elect to use certain products or services available on the Site. These may include: (a) registering for an account on our Site; (b) signing up for our newsletter subscription or special offers; (c) sending us an email message; (d) submitting your credit card or other payment information when ordering and purchasing products and services on our Site. Purchase and Treatment of Services: Minimum Age of 18
Think Fast PT requires that an individual purchasing any service through the company is at least 18 years of age. Any and all types of treatment, evaluation, consultation, or service provided by a Think Fast PT healthcare provider for an individual under the age of 18 must (A) be purchased by an adult who, for purposes of this Privacy Policy, is at least 18 years of age and (B) must be accompanied by an adult during the treatment, evaluation, consultation, or service rendered.

From time to time, Think Fast PT may contact you via email for the purpose of providing announcements, promotional offers, alerts, confirmations, surveys, and/or other general communication.
